She is survived by: her husband Curtis E. Cook; and her children, Curtis Dean Cook (Leisa) of Vernal, UT, Daniel Owen Cook (Marivi), Lisa Ann Larsen (Wayne) and Kristie Dawn Cook of West Jordan, UT.. She is also survived by 16 grandchildren and 6 great grandchildren.
