He is survived by: his parents, Evan John "Butch" Lively Sr. and Gloria Lively; his wife Gena; his children, Melissa Ann, Scott Nicolas and Richard B's; his grandchildren, Patience, Nikolas and Dyllan; his siblings, John Lively and Michelle Stone; and also Gloria Phillips. He is also survived by several nieces, nephews, aunts, uncles and cousins.
