He was predeceased by: his parents, Joseph Chaffin and Marjorie Chaffin; and his son Mark Allan Hazlett. He is survived by: his wife Carol of Buffalo; his daughter Deann Meyer of Buffalo; his brother Fred Hazlett of Casper; and his grandchildren, Hannah Brakebill of Buffalo, Jason Meyer of Buffalo and Kerrie Hazlett of Charlotte. He is also survived by six great-grandchildren.
Donations in Dean's memory may be made to the Suzie Bowling Hospice in care of the Harness Funeral Home at 351 N. Adams in Buffalo.
