He is survived by: his wife Naomi Ruth Perry (Headley); his grandchildren, Sarah (Gabriel Bruner), Elizabeth (Wade McNeal) and Benjamin Perry; his goddaughter Katrina Kolb; his close friends, Lenny and MariKay Wrabel; and also, Patrick, Connie Perry, Michael, Toni Perry, Richard, Vicki Perry, Jackie and Dave Bowser. He is also survived by three sons, a daughter.
Masks will be required and social distancing guidelines followed.
In lieu of flowers, the family requests that memorial donations be made in Jack's memory to the Westover Senior Center at 500 Dupont Rd. Westover, WV 26501 and Amedisys Hospice Care at 5006 Mid Atlantic Drive Morgantown, WV 26508.
