He was predeceased by: his parents, William W. Sr. Slade and Virgie L. Slade (Dallas); his son Kareem; his grandson Keonte; and his siblings, Rufus, Beverly, Evelyn and Sandra.

He is survived by: his wife Teresa A. Slade; his son Melvin A. Slade III; his siblings, William W. Slade Jr. (Patricia) of Princeton, WV, Garland R. Slade (Sharon) of North Carolina, Joanne Smith of Mt. Vernon, Vivian Stewart of Mt. Vernon and Barbara J. Deveaux of Mt. Vernon; and his close friends, Lonnie Parker, Conner Braggs, Lloyd Lipscomb, Mike Bishop and Erica Craighead. He is also survived by nephews, nieces and cousins; several brothers-in-law and sisters-in-law; his many friends.
