He is survived by: his father Roy Lewis (Alma) of Wayne, WV; his children, Ashley Marie Lewis of Huntington, WV, Michael Douglas Pack (Anna Smith) of Branchland, WV and Christopher John Lewis of Wayne, WV; his sister Renee Lewis of Wayne, WV; and his grandchildren, Gracelynn Renee Cassaday, Landyn Neil Cassaday, Kaiden D. Pack and Raylan D. Pack.
