He was predeceased by: his parents, Elmer Mayhorn Sr. and Marie Bryant Mayhorn; his wife Vanessa Mayhorn; his children, Joshua Blue Mayhorn and Shannon Mayhorn; and his siblings-in-law, Peggy Mayhorn, Marquita Brinager, Jack Mills, Ervin Williams, Alex, Bucci and Tony Carrico.
He is survived by: his son Cody Mayhorn; his siblings, Bill Mayhorn (Lona), Jane Mills, Eleanor Williams, Patty Smith (Challis), Judy Bowen (Dana) and Thomas Mayhorn; his sisters-in-law, Tammy Williams (D. J.), Cheryl Carrico and Donna Dingess (Clyde); and his nephew Billy Mayhorn (Sharon). He is also survived by several other nieces and nephews.
