He was predeceased by: his parents, Wayne Taylor Van Slyke and Gladys Lucille Van Slyke (Robertson); his wife Macie Van Slyke (Mowery); and his granddaughter Talia Kristen Mongold. He is survived by: his daughters, Jean P. Mongold of Mozer, WV and Nancy L. Cook (Darrell) of Woodstock, VA; his grandchildren, Jordan Wayne Mongold (Victoria), Tiffany Smith and Luke Smith; and his sisters, Vivian N. Dorville of Manteca, CA and Audrey L. Hawthorne of Auburn, CA. He is also survived by several nieces and nephews.
