He was predeceased by: his parents, Douglas Ray Cook and Wanda Lee Cook (Carter); his grandparents, Roscoe, Dona Cook (Bias), Ezra and Hattie Carter; and his sister Teresa Lynn Cook.
He is survived by: his wife Jeannie Lowry Cook; his daughter Sapphire Starkey (Caleb); his granddaughters, Adelyn and Emersyn Starkey; his siblings, Dr. Bridget Stevens and Victor Woods (Tara); and his pets, Wyatt, Chester, Millie, Odin and Luna. He is also survived by numerous nieces and nephews.
