He was predeceased by: his grandparents, Wade H. Jr., Wanda Coffindaffer, Jarrett D. (Pete) and Phyllis Fox; his uncle Ernie B. Fox; and his aunt Connie C. Ferrell.
He is survived by: his parent Wade H. III (Linda Fox Coffindaffer); his sisters, Sarah Tolley (Vince) of Spencer and Rebecca Gaughan of Spencer; his nieces and nephews, Brittany Smith, Cassidy Tolley, Katie Gaughan, Leah Tolley, Anna Tolley, Carter Gaughan and Evan Gaughan; his aunts, Deborah Fox of Spencer and Debra Oyler (Stacey) of Latrobe, PA; and his uncle William B. Ferrell Jr. of Jane Lew, WV.
In lieu of flowers, the family requests that contributions be made in Jay's memory to the Roane County Animal Shelter, 2550 Parkersburg Road, Spencer, West Virginia 25276.
