He is survived by: his parents, Wade Arthur and Debra Marlene Richards Eddy of Salem; his wife Tammy Smith Eddy; his children, Shelby Diane Eddy (William Pratt), Emily Raven Eddy and Zachery Michael Eddy; his grandsons, Christopher Paul Eddy, Noah Lee Eddy and Alistair William Pratt; his sisters, Shannon Eddy and Amber McGaughey (Kim); and his parents-in-law, Loren and Donna Smith.
