He is survived by: his wife Tonya Shelnutt Cook; his daughters, Kelsey Cook, Taylor Sutphin (Brett) and Saige Stafford (Elijah); his siblings, Charles Cook, Terri Adams (Bill) and Diane Painter (Stephen); his aunt Betty Williams; and his granddaughter Georgia "Peachy" Treadway. He is also survived by several nieces and nephews.
