She was predeceased by: her parents, Calvin Leslie Hayslett and Elva Ann Humphries Hayslett; her husband Charles Amick; and her siblings, Marietta Halstead, Marvel Hayslett, Mavis Hayslett and Melvin Ray Hayslett.
She is survived by: her children, Karen Adkins (Bob), Evelyn McCarter (Brandon), Angela Amick and Myron Amick (Rhonda); her grandchildren, Drew Ayers, Brent Amick and Caleb Levi Adkins; her brothers, Marshall, Marty, Henry and Mark; and her close friends, Mary Sue Lane and Annie Keeney.
