He was predeceased by: his parents, Robert Polk (Margarette) and Margarette Polk; and his brother Robert Polk. He is survived by: his daughters, Amy Phoenix (Michael) of Point Pleasant, WV and Carrie Polk of Illinois, Beloved; and his siblings, Barbara Smith (Joseph) of Point Pleasant, WV and Russell Polk (Roen) of Illinois, Beloved. He is also survived by his several grandchildren and great grandchildren.
