He was predeceased by: his parents, Theodore N. Ballard and Arline Wills Ballard; and his brother Joe Ballard. He is survived by: his wife Judy Smith Ballard; his children, Mick Ballard (Jill) of Peterstown, WV, Bob Ballard of Rich Creek, VA and Cassie Reed (Buck) of Peterstown, WV; his grandchildren, Cortney Tuggle (Jeremiah), Katie Buracker (Brett), Kelsey Reed, Caleb Ballard (Katie), Morgan Wickline (Chastin), Eli Ballard, Christian Ballard and Justin Ballard; and his great grandchildren, Kendall, Rylan, Kyleigh, Jaxton, Briggs, Lola, Quinn and Sheppard.
In lieu of flowers, the family requests that memorial contributions be made in Teddy's name to The Sports of Monroe Foundation at 280 Walnut Ridge Road, Peterstown, WV 24963.
