We are sad to announce that on June 21, 2020 we had to say goodbye to Gerald Maxwell Cook of Ottawa, West Virginia. You can send your sympathy in the guestbook provided and share it with the family.
He was predeceased by: his wife Phyllis; his parents, Holly Cook and Veda Cook of Ottawa, WV.; and his siblings, Faye Greene, Sharon Grant, Emil, Roy Serel and Arthur Dale Cook.
He is survived by: his children, Gary (Carla), Ronnie (Marcy), Danny (Georgann), David and Joyce (Brad); and his nieces, Vicky Guy (Larry) and Jeannie Price (Timmy). He is also survived by ten grandchildren; ten great-grandchildren; and two step-grandchildren; two sisters-in-law; numerous nieces and nephews; and a host of friends.
In lieu of flowers the family suggests making donations in memory of Max to Dignity Hospice PO Box 4304 Chapmanville, WV 25508.
