He was predeceased by: his parents, Otis Edward Smith, Sr. and Nettie Webb Smith; his wife Charlene Ann Smith; his sisters, Linda Smith and Deloris Smith; and his granddaughter Katherine McIntosh Toney.

He is survived by: his children, Shawn Smith of Mount Hope, Denise McIntosh of Beckley and Debra Watts (Mark) of Maple Fork; and his siblings, Carlos Smith, Billy Jean Smith (Deloris Smith), Bonnie Thompson (Kenneth) and Martha Mallahan (Eddie). He is also survived by five grandchildren and twelve great grandchildren.
