He is survived by: his parents, Philmore Keith Collins and Deloris Estelle Howell Collins of Adolph; his spouse Kelly Ford; his children, Whitney Warner (Gary) of Boyer and Joshua Collins of Mill Creek; his grandchildren, Gunner, Aubrey, Bailey, Avery and Owen; his siblings, Steven Ray Collins (Cathy) of Valley Bend and Debbie Barrickman (Tom) of Mill Creek; his siblings-in-law, Teri Howell (Bruce), Sheri Baker (Chip), Kerri Carr (Kelly), Chuck Ford (Sheila), Matt Ford and Marcus Blankenship; and his parents-in-law, Charles and Loretta Ford of Mill Creek. He is also survived by several aunts, uncles, cousins, nieces and nephews.
