He was predeceased by: his parents, Clarence Church and Gracie Church (Patterson); his wife Pat; and his siblings, Norman, Clyde, William, Mildred, Myrlie and Doris.
He is survived by: his sons, Mike (Tammy) of Grassy Meadows, Jason of Meadow Bridge and Leslie of Sam Black; and his siblings, Stanley of Jacksonville, Larry of Meadow Bridge, Patty of Danese and Marion of Mobile, Alabama. He is also survived by eight grandchildren; six great grandchildren; numerous nieces and nephews.
