He was predeceased by: his wife Louise Cook; his son Brian Cook Sr; his siblings, Charles, Gary King, Carolyn Beal and Diane Treadway; and his parents, Roy Cook and Wilma Gibson Cook.
He is survived by: his former wife Marilyn Cook; his children, Roy Cook II and Jennifer Cook; and his siblings, Glen, Richard King, Betty Hackney, Ella Blaha and Connie Withrow. He is also survived by six grandchildren and one great grandchild.
