He was predeceased by: his parents, Joseph Wellington and Dorothy Marie Cook (Spears); and his siblings, Elaine Brooks, Joseph W. Cook II and Charles "Chuck" Cook.
He is survived by: his wife Parthena Cook (Gibson) of Mallory; his sons, Zachary Scott "Zach" Cook of Mallory and Jason Tyler Cook (Amber) of Davin; his granddaughter Ariya Rachelle Cook "Poppy' (Davin); and his children, Shari Lynn Cook of Oceana and Shawn David Cook (Michele) of Oceana. He is also survived by other relatives, church family, and friends.
