He was predeceased by: his parents, John Calvin Forquer Sr. and Wynona Mae Hunt Forquer; his sister Barbara Gorby; his sister-in-law Becky Forquer; and his friends, Nancy Somers and George Morrison.
He is survived by: his wife Jan L Wagoner Forquer; his children, Zach (Marcey Forquer) of Clarksburg, Jesse (Jason Andriotto) of Clarksburg and Courtney Forquer (Brandon Smail) of Clarksburg; his grandchildren, Caitlin Forquer (Tommy Fluharty), Maddox Andriotto, Oliver Andriotto and Heath Thomas; and his siblings, Roy Forquer of Clarksburg, Cindy Forquer of Clarksburg and Darena Forquer of Clarksburg. He is also survived by many nieces, nephews and very close friends.
