He was predeceased by: his parents, Layton Blake and Clara Blake; his daughter Bonnie Clark; and his siblings, Murrel, Roger Blake and Lela Ingram.
He is survived by: his wife Patricia Blake; his son Shannon L. Blake (Mary Lou) of Lewisburg, WV; his grandchildren, Eric, Spencer, Stephanie and Sara; and his great grandchildren, Raelynn, Madilynn and Gunner.
