He was predeceased by: his parents, Joseph Polacek and Clara Urbassik Polacek; and his sister-in-law Anna Polacek. He is survived by: his wife Evelyn Rose Polacek; his children, Robert W. Polacek (Maria) and Susan Wheeler (Shawn); his grandchildren, Chrisandra, Alex, Katelyn Polacek and Marcie Wheeler; and his sister Elizabeth Jane Kelsey (Richard). He is also survived by nieces and nephews.
