He was predeceased by: his parents, Donald Sullivan and Garnett Petty Sullivan; and his brothers, Charlie, Donald, Larry and Richard Sullivan.
He is survived by: his wife Monica Brooks Sullivan of Huntington, WV; his children, Clarissa Blevins (Vernon) of Huntington, WV and Lucas Sullivan (Sonya Phillips) of Barboursville, WV; his sister Sandra Hay (Donald); and his grandchildren, Aurora Blevins, Damian Blevins, Alexis Sullivan and Dakota Sullivan.
In lieu of flowers, the family requests that memorial donations be made to the Barboursville Veterans Home.
