He was predeceased by: his parents, Henry Hazelett and Dee Kennedy Hazelett; his cousin Lil Stephens; and his brother-in-law Pat Ray. He is survived by: his wife Laura Gaskin Hazelett; his children, Evan Hazelett (Sara) and Shelby White; his grandchildren, Raegan and Wade White; his siblings-in-law, Jeff Gaskin (Anita), Greg Gaskin (Bev) and Peggy Ray; his nieces and nephews, Kristi Banks, Michael Gaskin, Shanna, Adam Gaskin and Andy Ray; and his pet Annie.
In lieu of flowers, contributions may be made to Mt. Union United Methodist Church.
