He was predeceased by: his grandparents, Ronald Polk, Mary Catherine Parcell, Opie and Romaine Sammons. He is survived by: his girlfriend Emily Daniels; his son Oliver Sammons; his parents, Mary Catherine Perry (Matt Perry) and David Wayne Sammons (Megan Sammons); his siblings, Sean (Hayden), Adam (Maggie), Jeremy, Stewart, Katie and Madison (Andy); and his grandparents, Shirley Stewart and Huey Perry (Jill Perry).
