He is survived by: his wife Lynn Calhoun Cook; his sons, BJ Cook (Jessica) of Gassaway and Heath Cook (Megan) of Gassaway; his grandchildren, Kolton Heath Cook, Katie Lynn Cook and Ella Elaine Cook; his sister DeLesa Belknap (Jack) of Charleston; his nephew Jimmy Belknap (Amber) of Scott Depot; his niece Jodi Lucas (Ben) of Charleston; his sister-in-law Alice James (Steve) of Gassaway; his nephew Isaac (Sheri Wang) of Pittsburgh; and his close friends, Buster Gum, Herbie Ratliff, Wayne "Newt" Shaver, Labradoodle and Daisy Doodle.
