He is survived by: his wife Arella Blankenship Underwood; his children, Patrick Underwood (Kesha) of Frankford, WV and Jennifer Harrah (Michael) of Asbury, WV; and his grandchildren, Ian, Madison Seal, Bryson, Kaelyn, Kyah, Sawyer and Slade Underwood. He is also survived by several nieces, and nephews.
