It is with great sadness that we announce the death of Leon Forman (Falling Waters, West Virginia), born in Boston, Massachusetts, who passed away on October 20, 2018, at the age of 79, leaving to mourn family and friends. He was predeceased by: his parent Elizabeth Beatrice Freeman Lazarus; his sister Beverly Goldman; his niece Ellen Rosen; and his sister-in-law Trudy Brody. He is survived by: his stepfather Irwin Lazarus; his wife Ruth Pauline Goldman Forman; his sons, Matthew D. Forman (Mary Ann) of Norman, OK and Jeremy S. Forman of Frederick, MD; his siblings, Albert Lazarus of Abu Dhabi, Stephen Forman (Elisa) of Brockton, MA and Felice Lazarus of Maynard, MA; his brothers-in-law, Martin Brody of Kings Park, NY and John Chadwick of Washington, DC.; his nephew Michael Brody; his children, Chase, Charlotte, Asher of Harleysville, PA, Julian Brody (Polly), Robbie, Jack of Southhampton, NY, Jonathan Lazarus (Katie), Emma and Dylan of Norwood, MA; his nieces, Miranda Chadwick (Kristy Noel) of Washington, DC, Celia Chadwick of Arlington, VA, Rebecca Chadwick (Scott Flaherty) of Brooklyn, NY and Michelle King (Tony); his son D. J. (Amber); and his grandson Felix of Janesville, WI.
