He was predeceased by: his parents, Luther P. Slack and Madeline Slack (Moles); his son Robert William Slack (Robbie); his brothers, Larry (Tamara Slack) and Henry; and his great-niece Christina Slack.
He is survived by: his wife Patricia Ann Slack (Jones); his son Eric W. Slack (Lora); his siblings, Ray (Darlene) and Sue Facemyer (Eugene); and his sons, Scott Slack and Greg Taylor. He is also survived by extended family and friends.
