He is survived by: his brother Emmit Jack Bolen; his son Johnny Kim Bolen; his wife Alice Arbutus; his daughter Faye; his son-in-law Brian Shafer of Frame; his son Jack Alex of Elkview; his grandchildren, Billy (Rachel) of Cleveland Ohio and Cristina (Jamin) of Scott Depot; and his son Jerry Gaylor.
