She was predeceased by: her parents, Steve Hornick and Annie Semonco Hornick; and her husband Fredrick Slaymaker. She is survived by: her son David Mellert; her sister Helen Burnop; and her niece Karen Blizzard. She is also survived by numerous nieces and nephews.
