He was predeceased by: his parents, Adam Kukoleck and Marie Lane Kukoleck; and his wife Donna Morris Kukoleck. He is survived by: his sons, Adam Kukoleck (Wendy) and David Michael Kukoleck (Carla); his grandchildren, Josiah David Kukoleck and Jedediah Michael Kukoleck; and his sister Sharon Rice (Ken). He is also survived by several nieces, nephews, great-nieces, great-nephews, one great-great-niece; and many extended family and friends.
