He was predeceased by: his parents, Beaulis Lively and Faye Lively (Tyree); and his brother Perry Lively. He is survived by: his wife Cheryl; his children, Richard Lively (Shona) of Wayne, WV and Tammy Thomas (Tom) of Lavelette, WV; and his stepchildren, Robert Cobb (Lindsey) of Redhouse, WV, Joshua Cobb of Ashland, KY. and Chasity Fryer (Matt) of Ashland, KY..
