He was predeceased by: his adoptive parents, Okey Thomas and Sylvia Hazel Kincaid Cook; and his brother Okey Thomas Cook Jr.. He is survived by: his parents, Brian Tuma and Darlene Cook Israel of Coal City; his siblings, Glenn Michael Israel of Coal City and Sherry Bolen (Chris) of Coal City; and his aunt Beverly Evans (Mason) of Coal City. He is also survived by several nieces, nephews, cousins and friends.
