He was predeceased by: his parents, Donald Henry and Pauline Grant Lewis; and his siblings, Judy Prout and Donald Lewis. He is survived by: his wife Judy Kay Blair Lewis; his children, Jerry Wayne Lewis II (Beverly), Christopher "Chris" Wayne Lewis (Kristina) and Cody Allen Lewis; his grandchildren, Xavier, Chloe, Brian, Jakobe, Conner and Kaydeelyn; and his siblings, Donna Jean Forge (Cleveland), Patty Lewis (Dave Smith), Cheryl Beatty (Gary), Diane Jackson (Kenny), Ronnie Lewis of Las Vegas, NV and Ricky Lewis. He is also survived by several nieces and nephews.
