He was predeceased by: his parents, Jerry Booth and Karen Booth. He is survived by: his wife Chassidy; his siblings, Jerry (Pam), Tracy and Randy (Rhonda); and his children, Clay, Kelly, Dalton, Hayley, Allie and Jerrika Jo. He is also survived by 3 grandchildren.
