He is survived by: his wife Lisa West Cook; his children, Josh Cook (Mallory) of Bridgeport and Molly Cook of Bridgeport; and his brothers, Robert Cook (Jinah) of Scott Depot and David Cook (Kerrie) of Bridgeport. He is also survived by several nieces and nephews.
