He is survived by: his wife Donna Cook (Meeks); his children, Joseph, Jemma and Jonathan Cook; his siblings, Judith Sullivan (Frank) of Jacksonville, FL, Teresa Winfield (Robert), Thomas Cook (Pam) of Charlotte, NC, Timothy Cook (Erin) of Coral Gables, FL, Mary Cook of Jacksonville, FL, Patricia Stiles (Raymond) of Jacksonville, FL and Barbara Sutherland (David); and his brother-in-law Rick Harrold.
