Bolen (Smith); his stepson Stacy Johnson (Lori) of Crab Orchard; his grandchildren, Anthony Johnson (April McKinney) of Beckley and Erica Johnson of Beckley; his great grandchildren, Kasen of Beckley, Annabel of Beckley and Liam of Beckley; and his siblings, Phyllis Mustafo of Beckley, Roger (Debbie Bolen), Thomas Bolen of Coal City and Dwayne Bolen of Jonben. He is also survived by nieces, nephews, great-nieces and nephews; and friends.
