He was predeceased by: his parents, Kermit Qualls and Lois Clark Qualls; his wife Joy Bunn Qualls; and his brother Jack Qualls. He is survived by: his children, Johnathan Mark, Arlene Qualls of Huntington, Michael "Chuck", Maguana Qualls of Carrolton, KY, Cathy and Mike Offill of Olive Hill, KY. He is also survived by five grandchildren, five great-grandchildren and several nieces, nephews and extended family.
