He was predeceased by: his parents, John Booth and Beulah Booth; his sister Carol Casto; his brother-in-law Jim Phillips; and his nephew Tim Casto.
He is survived by: his wife Nancy Parsons; his son Dr. Rick Booth (Angela Ferguson) of Huntington; his grandchildren, J. T., Emily and Luke; and his siblings, Ruth Arthur (Roosevelt), Betty Phillips and Johnny Booth (Mona).
In lieu of flowers the family requests that donations be made to Kanawha Hospice.
