She was predeceased by: her parents, Russell Moore and Josephine Moore (Hancock); and her siblings, Wayne, Lucille, Doris and Carol.
She is survived by: her husband Donald Fornasiere; her children, Robin Fornasiere (Ruth) of Maple Grove, MN, Susan Suchomel (David) of Sun Prairie and Karen Budde (Tom) of Grafton; her grandchildren, Andrew, Matthew, Katie, Mark, Timothy and Christine; and her great grandchildren, Calan, Case, Colette, Parker and Adelyn.
In lieu of flowers memorials may be designated to Lakes Area Christian Fellowship or Alzheimer's Association.
