He was predeceased by: his parents, Gerald Falk and Bonnie Falk (Arneson); and his father-in-law Richard Vetter. He is survived by: his wife Lisa; his sons, Erik Falk (Becky), Adam Falk (Miranda) and Peter Falk (Amy); his grandchildren, Brayden, Aaron, Evan, Eli and Lydia; his sisters, Jaci Lange (Mike) and Jodi Kingsfield (Steve); his mother-in-law Edna Vetter; and his sisters-in-law, Anne Truniger (Dominic), Mary Wade (Gary), Jody Vetter (Neil) and Penny Moran (Don). He is also survived by many nieces and nephews.
In lieu of flowers, memorials can be made in Jerry's name to Washington Elementary School in Wisconsin Rapids, WI, or St. Thomas Aquinas school in Avondale, Arizona.
