She was predeceased by: her parents, Leon Kubisiak and Sophie Kubisiak (Borchardt); her brothers, Leo Kubisiak and James Kubisiak; her brother-in-law John Koltz; and her niece Lisa Brooks.
She is survived by: her sister Shirley Koltz; her nieces and nephews, Cindy Sawtell (Gary), Allen Kubisiak (Andrea), Matthew Koltz, Judy Grover (Alan), Joe Kubisiak (Michelle), Bob Kubisiak (Tina), Ned Kubisiak (Kami), Christopher Koltz (Traci), Thad Kubisiak and Justin Kubisiak (Jessica); and her sisters-in-law, Kris Kubisiak and Peggy Kubisiak.
