He was predeceased by: his son Jimmy; and his parents, Harold Sprague and Amy Sprague. He is survived by: his wife Carol Belke Johnson; his grandsons, Tarek, Teagan Herschleb, Noah and Joseph Cook; his daughter-in-law Jodi Underwood; his goddaughter Taralyn Shimel; his siblings, Harold, Edward, Laurie and Linda Braun (David); his parents-in-law, Donna, Larry Holl, Richard, Sue Belke, Loretta and Neil Belke; and also Jim Cook. He is also survived by many cousins, nieces and nephews.
