He was predeceased by: his mother Nancy Holzem (Kozak); his grandparents, Marge, Marvin and Pat Kozak; and his uncle Scott.
He is survived by: his father Brian Holzem (Donna); his wife Megan; his children, Madison and Liam; his sisters, Katherine (Matt) and Kassandra (Tim); his nephews, Henry and Leland; his grandmother Florence Kozak; his mother-in-law Becky Jarzynski (Rob); and his brothers-in-law, Jake and Taylor. He is also survived by his many Uncles, Aunt, and Cousins.
