He is survived by: his wife Janice L. Prochaska; his son Keith Prochaska; his granddaughter Jennifer L. Baum; his sister Patricia Frank; his siblings-in-law, Carol Prochaska, Jacqueline Kortendick, John, Elyse Adams, Karen Adams and Carol Adams; his pet Brett; and his caregivers, Liz, Anthony, Tracey and Carissa. He is also survived by nieces, nephews, other relatives and friends.
