He was predeceased by: his parents, Thomas Francis Fay and Mae Catherine Fay. He is survived by: his son Joe Fay (Kimberly W.); his siblings, Kathy Wenzlaff (Gene), Kevin Fay (Chris) and Kerry Patrick Fay; his nieces and nephews, Holly Feucht (Steve), Shannon Schill, Carissa Jacox (Mark), Barry Wenzlaff (Renee), Shad Wenzlaff, Korey Fay and Colton Fay (Jaclyn); and also Zeb. He is also survived by other relatives and friends.

The family asks that if those who wish to donate any funds to please do so to the Boltonville Sportsman Club in Tom's honor.
